1. The Opportunity
The Lōkahi Healthcare Accelerator represents a transformative opportunity to unify Hawaii’s health ecosystem. Spearheaded by the EP3 Foundation and the Hawaii State Health Planning & Development Agency—with support from Governor Josh Green, the legislature, and a broad coalition of partners—Lōkahi will harness Privacy Networks and the Privacy Network Exchange to integrate care delivery, payments, public health, patient safety, and clinical research. The result: a privacy-preserving, AI-optimized personalized health marketplace that drives better outcomes and greater value for all.
What This Looks Like in Practice
Here's what privacy-preserving healthcare means for real people in Hawaii:
Coordinated Care Without Data Silos
A patient sees a specialist on Oahu, a primary care doctor on Maui, and fills prescriptions at a pharmacy on the Big Island. Today, none of those providers can see each other's records without the patient carrying paperwork or signing new consent forms at each visit. With Lōkahi's privacy-preserving infrastructure, the patient's AI agent shares exactly what each provider needs — lab results with the specialist, medication history with the pharmacist — without any provider seeing the full record, and without a centralized database that could be breached.
Population Health Without Surveillance
Public health officials track disease patterns across the entire state — by age, geography, and risk factor — without ever seeing an individual patient's identity or medical record. The Privacy Network delivers population-level insights from individual-level data, with cryptographic guarantees that no individual can be re-identified.
2. Our Mission
The goal is to incubate innovation that improves healthcare outcomes and reduces costs while expanding access for underserved and rural populations. The initiative provides for common coverage requirements, risk pooling, and value-sharing, and covers the entire population including patients covered by Medicare Advantage, Medicaid, state, employer-sponsored and commercial plans, the VA, and safety net programs.
3. Infrastructure & Governance
Lōkahi runs on production-grade infrastructure operated by Milliman on Databricks and Azure — inheriting existing SOC 2, HIPAA, and financial compliance controls from day one. Governance is provided through the EP3 Trusted Network Accreditation Program (TNAP), which certifies that participating systems meet privacy, cybersecurity, and regulatory compliance standards. WebShield provides the privacy architecture layer — consent schemas, interoperability patterns, and Privacy Network overlays — ensuring all participants build to the same standards.
